Integrative Elder Care RN ​offers comprehensive care coordination and planning for elders and their families, looking at the person's whole life, including health challenges, safety, quality of life and working with what matters most in life to him or her. In collaboration with family or friends, health care practitioners and community resources, we create and carry out personalized plans for physical, spiritual, emotional and cognitive health in the most secure and comfortable environment possible. Each person's needs and preferences are different and vary with time, so care evolves as conditions improve or change. We work with you and a network of experienced and creative professionals, caregivers and agencies to build a well-matched team for short or long term care, with regular visits and communications with the family, the guardian or the responsible individual.

Integrative health means combining forms of healing practices and conventional medicine to best support health, comfort and overall well-being.

This includes the best of science and medicine, working with medical treatment regimens, and when appropriate, a range of mind-body practices including food as medicine, exercise, spiritual support and enjoying the arts and intellectual activities. All are coordinated with consideration of the medical plan and the elders personal preferences, energy, abilities, resources, family and community involvement and in consultation with the elders medical team.

Care Coordination and Nursing With a Holistic and Collaborative Approach

  • Assessments and written recommendations for home care needs, home safety and resources
  • Creation of a Care Plan, regular visits and coordination of care in transitions of health. abilities cognitive levels or relocation
  • Referrals to and liaison with medical practitioners, hospital, therapists and home care agencies
  • Advocacy for the elder at appointments with health care providers or in hospital stays
  • Oversight, training and support for Care Partners in the home
  • Phone Consultation for elders, families, long distance caregivers
  • Ongoing collaboration with the elder, family, friends, physicians, nurses, social workers, therapists, nutritionists, clergy and community members with verbal and written communication and reports.
  • Promoting the understanding of health conditions, regimens, and treatments, respecting the elder's cultural traditions and choices
  • Conserving of resources, energy, and funds for what matters most to the elder. creatively finding their balance
  • Referrals to social services. elder law attorneys. financial professionals and community resources
  • PRI and SCREEN (certified assessor ) for New York nursing home admission
